Can I set up a tiered/ladder program based on amount of orders or total sales?

Yes. Use performance-based ladder rules: define tiers, then rules that promote affiliates when they hit a threshold (e.g. X amount approved orders or $X approved sales). They’re moved to the higher tier automatically.
